New Service

EXISTING SERVICE - RESIDENTIAL

Please fill out the applicable forms , bring the forms in to the business office along with your connection fee and proper forms of identification.

Forms that are applicable for new existing residential service:

Application (front and back)
Cross Connection Control Device Installation Agreement (you will need to fill out this form if you have a sprinkler meter)

EXISTING SERVICE - COMMERCIAL

Please fill out the applicable forms , bring the forms in to the business office along with your connection fee and proper forms of identification.

Forms that are applicable for new existing commercial service:

Application
Cross Connection Control Device Installation Agreement (you will need to fill out this form for each water/sprinkler meter)

NEW SERVICE - RESIDENTIAL

Please fill out the applicable forms , bring the forms in to the business office along with your connection fees, tap fees and proper forms of identification.

Forms that are applicable for new residential service:

Application (front and back)
Cross Connection Control Device Installation Agreement (you will need to fill out this form if you have a sprinkler meter)
Excess Flow Valve Contract(you will need to fill out this form if you will be applying for natural gas service)

**You will also need to bring a Building Permit that you can obtain from the City of Gallatin Codes Department located in City Hall.

NEW SERVICE - SMALL COMMERCIAL

Please fill out the applicable forms , bring the forms in to the business office along with your connection fees, tap fees and proper forms of identification.

Forms that are applicable for new small business service:

Application
Cross Connection Control Device Installation Agreement (you will need to fill out this form for each water/sprinkler meter)

**You will also need to bring a Building Permit that you can obtain from the City of Gallatin Codes Department located in City Hall.

NEW SERVICE - COMMERCIAL/INDUSTRIAL

You will need to submit your plans to the Department for review and approval. Upon review and approval of your submitted plans a Fee Schedule will be generated. You will need to submit the Fee Schedule along with the same forms applicable for Small Commercial and your connection fees and tap fees, to the business office.
